The reputation is forty years out of date. The tooth is numbed exactly as it would be for a filling, and the pain people remember is the infection, which this is what removes.
The nerve space is cleaned, shaped and sealed. Back teeth have three or four canals and take longer than front teeth — plan for ninety minutes, sometimes over two visits.
A treated tooth is more brittle than a live one, so a back tooth almost always needs a crown afterwards to stop it splitting. That is a separate appointment and a separate cost, and you will be told about it before the root treatment starts, not after.
